Terms of Service for SMS Text Messaging Effective Date: October 1st, 2024 These Terms of Service govern the use of SMS text messaging communications provided by UtahSurgery (“we,” “us,” or “the Practice”). By opting in to receive text messages from our clinic, you agree to the terms outlined below. UtahSurgery offers SMS messaging as a convenience to patients for purposes such as appointment reminders, scheduling updates, and other care-related notifications. Participation in SMS messaging is voluntary, and you are not required to opt in as a condition of receiving care. When you provide your mobile phone number and consent to receive SMS messages, you authorize UtahSurgery to send text messages to that number. Message frequency may vary depending on your appointments, care needs, and communication preferences. Text messaging is primarily intended for administrative and care coordination purposes. While we take patient privacy seriously and follow applicable HIPAA standards, standard SMS messaging is not always a fully secure form of communication. For this reason, UtahSurgery will not send highly sensitive medical information through text. Patients with urgent or detailed medical concerns should contact our office directly by phone. You may opt out of receiving SMS messages at any time by replying STOP to any message. Once you opt out, you will no longer receive further text communications unless you choose to re-enroll. If you need assistance, you may reply HELP for support. Standard message and data rates may apply depending on your mobile carrier and plan. UtahSurgery is not responsible for any charges incurred by your wireless provider as a result of SMS communications. We respect your privacy. Mobile numbers and SMS opt-in information will be used only for communications between you and our clinic. We do not sell, rent, or share your opt-in information with third parties or affiliates for marketing purposes. UtahSurgery may use third-party service providers, such as ActiveCampaign, to facilitate the delivery of SMS messages. These providers are permitted to use patient information only as necessary to provide messaging services on our behalf and are required to maintain appropriate safeguards. By opting in, you acknowledge and accept these Terms of Service. UtahSurgery reserves the right to update or modify these terms at any time.
